Before filling out the Facility Request application form, please carefully read through the usage policies effective March 29, 2026 located above this form. If you have any questions, please do not hesitate to contact the church office.
Once the application is complete, a representative from the church office will confirm your request and send additional information for your event. Your booking is not completed until you receive confirmation from the office.
